File a Complaint

This guide will walk you thru filing a complaint. Please read the steps below and then choose the method most convenient for you to file a complaint.

1. Review the Citizen Complaint Procedure Guide

2. Identify the officer(s) or employee(s) involved

There are a few things that will help us to best investigate your complaint.

Employee’s or Employees’ Names*
Employee’s or Employees’ Badge Numbers*
Information that can assist identifying the employee
Names and Information for Witnesses of the Incident

*Note: you can file a complaint without names or badge numbers. These are preferred but not required.

3. Prepare a description of your incident

It helps to be as detailed as possible. Any information you can provide will help our investigation. We’ll provide a space for this description in the form, but you can attach additional pages if needed.
